MAMS: Merchant Waiver for Rep Approval

If your merchant would like to sign a waiver allowing us to take requests/approvals from rep’s on their behalf, please send them this link.

PLEASE DO NOT SEND THIS FORM TO THE REPS: This will help eliminate potential fraud (aka reps filling out the forms as the merchant themselves) If you suspect any fraud, please alert a supervisor before proceeding.

Once submitted, it will be sent to [email protected]. Please save this form into the merchant’s specific folder on the server. Please create a folder titled “Maintenance” if one does not exist already. Then create a folder titled, “Merchant Waiver for Rep Approval”, and drop the wufoo form into that folder.

Then please note in the Appflow description box in the following format:

MERCHANT WAIVER FOR REP APPROVAL:

• [DATE]: Merchant was sent waiver
• [DATE]: Merchant submitted waiver for [Insert Rep’s Name Here]

PLEASE NOTE: If a Rep changes and you need an updated form, please simply resend the form, save second form with the previous form, and make additional notes in Appflow.
