Apple Developer Account Form/Template for Merchant
There are three different templates to choose from. General, 30 Day, and Expired. If Apple has pulled the App due to non-compliance, please use the Expired template. If Apple has given us a 30 Day notice, please use the 30 Day template, if there is no rush, please use the General template.

Previously, we, as Total Loyalty Solutions, were able to house your App in iTunes under our own Apple Developer Account, however, Apple is now requiring that each business reside in its own Apple Developer Account. This means that you will need to enroll for and purchase an Apple Developer Account in order for iTunes to continue to house your App and for TLS to update/manage your App in iTunes. We have provided an overview below of what you will need to do with Apple to complete this process. Please continue to scroll for more detailed instructions, links, and helpful tips. IMPORTANT NOTE: The Apple Developer Account setup process must be handled solely by the owner of the business. Unfortunately, assistants, family members, spouses, or other employees of the business are not able to be appointed by the owner as the communicator or manager of this process. This is a strict requirement and security guideline of Apple.

STEP #1: CREATE AN APPLE ID
YOU WILL NEED:
- Email Address: You will need to have access to this email inbox to retrieve verification codes/updates. This email address cannot be associated with another personal AppleID.
- Phone Number: You will need to have access to this number to receive calls/verification codes.

STEP #2: CHOOSE TO ENROLL AS AN “INDIVIDUAL” OR AN “ORGANIZATION”
You will need to choose whether you would like to enroll for an Apple Developer account as an Individual (Sole Proprietor) or an Organization (Business Entity). The only difference between these two options is the way the name appears within the detailed view of the App in iTunes. For Sole Proprietor, you will see the owner’s personal name as the account owner. For Business Entity you will see the business name as the account owner. (See below for visual example)
IMPORTANT NOTE FOR BUSINESS ENTITY OPTION:
The Business Entity option requires that you have BOTH a business email address (ie; [email protected]) (cannot be @gmail, yahoo, aol etc) AND a registered DUNS # with Dun & Bradstreet. If you do not have either you will need to obtain before submitting or choose Sole Proprietor. To obtain a business email address, you would need to reach out to your web developer. To find, obtain, or register a DUNS#, you would need to search https://www.dnb.com/duns-number/lookup.html or reach out to Dun & Bradstreet directly. Please note that Business Entity accounts can continue to be used in the case of new ownership whereas a new owner would be required to set up a new Apple developer account if the old owner has a Sole Proprietor.

STEP #3: ENROLL & PURCHASE
YOU WILL NEED:
- APPLE ID: created above
- DRIVER’S LICENSE & PROOF OF OWNERSHIP: You will need to upload a clear picture of the owner’s driver’s license. The ID must be current (IT CANNOT BE EXPIRED) and the name on the driver’s license must match the name you provided. You may also be required to upload Proof of Ownership documentation. In this case, these documents must be CURRENT, LEGALLY BINDING, INCLUDE THE NAME OF THE BUSINESS, THE OWNER’S NAME, AND PROVE LEGAL OWNERSHIP. (ie; liquor/business license, sales tax certificate, etc.) Apple requires this information as an extra security measure to ensure that you are the owner of the business that is requesting an App be placed into iTunes. This helps make sure that others cannot falsely claim ownership of your business. This information is private to Apple and is not publicly shared.
- FORM OF PAYMENT: The Cost of the Apple developer account is an annual fee of $99.00. You will be alerted to renew annually. Please note that Apple will remove your App from iTunes if your Apple Developer Account expires.

WHAT YOU CAN EXPECT:
Apple will let you know via email of any hiccups along the way, like if they need a clearer driver’s license, different proof of ownership, or even if the legal entity name and DUNS do not match up. Apple will also let you know if they need to speak with you to confirm owner identity. Apple will also let you know if they reject the request and why. If there are any bumps in the road, you will work together with Apple to appease Apple’s requirements. (If you have any questions about any of Apple’s communication with you, please feel free to reach out to us for clarification on what may be needed and we will help as much as we can)
Account requests are time sensitive. If too much time passes without acquiring what is needed for Apple to continue processing the request, it is possible for them to cancel the request. If they cancel the request, you will need to start over and resubmit a brand new request to Apple. Be diligent about following up for what you need to move forward.

STEP #4: ASSIGN TLS AS AN ADMIN
Once your account is complete, you (as the account holder) will need to assign TLS as an Admin role within your account so that TLS can continue to update/manage your App in iTunes. You will need to add two TLS Admins; their information is below. These users will need added as “Admins” not “Developers” and need to be granted “Access to Certificates, Identifiers & Profiles”. Once TLS is alerted of access, we will transfer your App into your Apple Developer Account and regular maintenance of your App in iTunes will continue.
